Medicare Patients: The optometrists at this office have elected to opt-out of Medicare, as of March 1st, 2017 as permitted by Section 1802(b) of the Social Security act. If you have Medicare, and are seen for eye care at this office, you will be billed the usual & customary rates. By consenting to treatment/services at our office, you agree that you shall not submit a claim for payment to Medicare for our services, nor shall we submit a claim or bill to Medicare, even if such services would otherwise be covered by Medicare (routine glasses/contact lens exams are typically NOT covered by Medicare). You will be fully responsible for the payments of all services rendered on the date of service, through private insurance or otherwise.

If you currently have vision insurance that is not listed above, please contact your insurance company to ask about using 'out of network benefits' to be reimbursed for services at Brown Vision Care.
